Q. Do I need to bring anything to the physical exam?
A. Yes. Please fill out the history portion of the physical form prior to the exam. The physical form must be filled out by a parent and signed prior to the physical.

Q. How much does the exam cost?
A. The cost may vary, but is is normally the same as most co-pays. Contact the host school for cost.

Dear parent,
____________ High School is hosting on-site sports physicals for all students interested in participating in the upcoming athletics season. This is a requirement for all student-athletes, regardless of year in school or participation level. A new physical is required each school year, and students not current will be ineligible to join their teams at the season’s start.

Nothing could be more convenient for you or your student-athlete. No appointment is necessary, wait times are extremely short and privacy is ensured. Male and female physicians are also available. And if that isn’t enough incentive, a portion of the proceeds benefits our own athletic department.
